If you would like to make an appointment with one of our practitioners, please contact our reception staff. Alternatively you may book your appointments online. Every effort will be made to accommodate your preferred time and choice of practitioner.
To get your copy of medical records you can call to front desk receptionist or billing services. As all the records are kept electronically in system you can receive the copy on your HIPPAcompliant email or fax.
You should bring copies of all pertinent medical records such as doctors’ notes and laboratory and imaging results, including actual films, active insurance card & photo id proof. This information helps your doctor to understand your medical history, make a diagnosis, and suggest treatment
options.
When you schedule your visit, our appointment coordinators will tell you when you should arrive and how you can prepare. In general, please plan on arriving 15 minutes prior to your scheduled appointment time.
